Abstract:Leprosy is an infectious disease caused by Mycobacterium leprae, a bacterium which primarily affects the nerves and skin. Worldwide 513,798 new patients were detected in 2003. Trend analysis showed that the new case detection rate did not decline between 1985 and 2000. Leprosy diagnosis is aminly based on the presence of skin lesions, loss of sensitivity and thickened nerves. The clinical pattern varies widely between patients due to a different level of celular immunity that an infected person mounts to the infection. For treatment purposes patients are divided into paucibacillary (PB) and multibacillary (MB) patients. Effective treatment against leprosy is available (chapter 1). In the last five decades cohort studies have investigated risk factors for developing leprosy. Most of these studies were conducted among contacts of leprosy patients and a few were population-based. Risk factors for developing leprosy were grouped into three levels: (1) individual level; (2) household and community levels; (3) meso/macro levels (chapter 1). As leprosy case detections rates did not decline in the last 15 years despite effective multi-drug treatment, new approaches and strategies to definitely eliminate leprosy as a public health problem are required. Four research questions were addressed in this thesis: "Which patients transmit leprosy?", "What are risk factors for developing leprosy?", "Can rifampicin, used as chemoprophylaxis, prevent leprosy?" and "How can chemoprophylaxis be most efficiently implemented?". All studies presented in this thesis were preformed in the same study area, five geographically isolated islands in the Flores Sea, Indonesia, and are part of still continuing population-based cohort study. Chaper 2 mainly serves as background information on the leprosy epidemic in the study area. Abstract:The molecular events that occur at the early phase of many demyelinating neurodegenerative diseases are unknown. A recent demonstratation of rapid demyelination and axonal injury induced by Mycobacterium leprae provides a model for elucidating the molecular events of early nerve degeneration which might be common to neurodegenerative diseases of both infectious origin and unknown etiology. The identification of the M. leprae-targeted Schwann cell receptor, dystroglycan, and its associated molecules in myelination, demyelination and axonal functions suggests a role for these molecules in early nerve degeneration (AU).

Abstract:Leprosy remains an important health problem wordwide. The disease in caused by a chronic granulomatous infection of the skin and peripheral nerves with Mycobacterium leprae. The clinical range from tuberculoid to lepromatous leprosy is a result of variation in the cellular immune response to the mycobacterium. The resulting impairment of nerve fuinction causes be disabilities associated with leprosy. This review summarises recent advances in understanding of the biology of leprosy, clinical features of the disease, the current diagnostic criteria, and the new approaches to treatment of the infection and the immune-mediated complications. Supervesed multi-drug therapy (MDT) for fixed durations is highly effective for all forms of the disease. The widespread implemantation of MDT has been associated with a fall in the prevalence of the leprosy but as yet no reduction in the case-detection rate globally. Thus, leprosy control activities must be maintained for decades to interrupt transmission of infection (AU).

Abstract:Analisou-se a prevalencia de anticoepos IgM utilizando teste sorologico e antigeno especifico Glicolipidio Fenolico I (PGL-I) do Mycobacterium leprae, por meio do teste rapido ML Dipstick em 1611 individuos sadios categorizados em contatos de pacientes hansenicos intradomiciliares e comunitarios, residentes em cinco municipios endemicos para hanseniase. Realizou-se estudo tranversal pela analise do banco de dados do laboratorio de hanseniase, Instituto Evandro Chagas. O estudo estendeu-se de março a dezembro de 1998, com reavaliaçoes anuais ate dezembro de 2002, no qual, os municipios selecionados-Curinopolis, Eldorado do Carajas, Xinguara, Rondon do Para e Dom Eliseu - apresentaram taxas de prevalencia em hanseniase maiores que 20 em cada 10.000 hab. e detecçao de casos novos maiores que quatro em cada 10.000 hab. A idade variou de 2 a 90 anos, devido ao fato de que a doença em areas de alta endemicidade possibilita o diagnostico de crianças. Estudou-se 957 mulheres (957/1611-59.4 por cento), 654 homens (654/1611-40.6 por cento) e observou-se predominancia de soropositividade entre as mulheres na faixa etaria entre 21 e 50 anos, 62.81 por cento contra 53.97 por cento nos homens. A analise foi realizada nos bancos de dados D-BASE, EPI-INFO 6.4 e 2002, BIOESTAT 3.0 e SINAM, existentes no Instituto Evandro Chagas e Secretaria Estadual de Saude do Para...(AU).

Abstract:Se realizó un estudio mediante pruebas cutáneas con antígeno soluble de M. leprae (ASMI y derivado purificado de porteínas (PPD) en 192 convivientes de pacientes de lepra multibacilar, con el objetivo de evaluar la utilidad del método para identificación de indivíduos presumiblemente infectados com M. leprae. Todos ellos residían en el municipio Guantánamo, que es el área de más alta prevalencia de lepra en Cuba. El tamaño medio de la reacción fue 5'03 mm. mientras que en los grupos de control utilizado fue 0 mm. en pacientes lepromatosos, 18'62 mm. en pacientes tuberculoide, 2'75 mm. en pacientes tuberculosis pulmonar y 1'72 en individuos sanos. El criterio de positividad para ambas pruebas fue de 6 mm. Se observó que la prueba cutánea con ASMI puede der útil para al conducción de estudios epidemiológicos ya que, entre los convivientes positivos, el tamaño de la reacción fue significativamente mayor que el observado con el PPD. Abstract:Basándose en el concepto de que "vacunar contra la lepra no es inocular B.C.G., sino comproblar que Mitsuda ha virado", los autores exponen sus experiencias con los 4 siguientes grupos de sujetos. Grupo I: 28 pacientes lepromatosos (LL), Mitsuda negativos. Grupo II: 10 pacientes tuberculoides (LT), Mitsuda positivos. Grupo III: 7 contactos (niños de 7 a 14 años de edad), Mitsuda negativos. Grupo IV: 26 contactos (niños de 6 a 15 años de edad), Mitsuda negativos. A los integrantes de los primeros 3 grupos mencionados se les aplicó el Antígeno Zulia, constituido por una suspensión bacilar de micobacterias aisladas en cultivo a partir de pacientes lepromatosos (0'1 ml. por vía intradérmica), y simultáneamente, o de 1 a 3 meses después, se les sometió a la prueba clásica de Mitsuda. A los contactos reunidos en el Grupo IV y ensayados con fines comparativos, se les aplicó B.C.G., en substitución del Antígeno Zulia. Estos ensayos demostraron que en 24 de los 28 pacientes L.L. (Grupo I), y en 5 de los 7 contactos (Grupo III, todos inicialmente Mitsuda negativos, se presentó un inminente viraje de los valores inmunológicos, fenómeno inducido por la aplicación del Antígeno Zulia y manifesto por la aparición de reacciones positivas a la lepromina clásica. Por otra parte se hizo notorio que de los 26 contactos vacunados con la única dosis de B.C.G., solamente 14 habían virado su Mitsuda a positivo. Los autores suponen que el "virage" mencionado es una expresión de mecanismos defensivos en individuos en los cuales éstos estaban bloqueados (probablemente por desaparisión de la capacidad linfocitaria de producir el factor de migración de macrófagos) y señalan que el Antígeno Zulia se destaca ampliamente por su capacidad de convertir a los sujetos Mitsuda negativos en sujetos Mitsuda positivos.
